CLINICAL FEATURES : 6 months male baby presented with cry.

SONOGRAPHIC  FINDINGS :   Mass with hypoechoic rim with central echogenicity. (pseudokidney sign)  at transverse, desending colon, rectal  regions. 

DIAGNOSIS : Ileo-colic intussusception is per -operative diagnosis.

DISCUSSION : Intussuscetion is the most common cause of small bowel obstruction in chindren between the ages 6 months and  4years. Clinical findings of crampy,intermittent abdominalpain, vomitings and current jelly stools are classic nature.Resent studies shows that ultrasound is highly sensitive and specific for the diagnosis of  intussuception.

                        Sonographically intussusception appear as an oval, hypoechoic with central bright echoes on longitudinal imaging(i.e. Pseudokidney sign) and target confugaration on cross sectional image.Hypoechoic rim represent s the edematous wall of the intussusceptum and central echogenicity represents compressed mesentary , mucosaand intestinal contents.Large amount of fluid in peritonium suggestive of bowel perferation.(Small amount of ascitis may present in peritonium even without peforation.)

Role of sonography is to establish diagnosis , to detect complecations,  to guide the mode of treatment, to assess the response for  non-surgecal treatment, ? to assess the risk of perforation by color doppler.
